MEDAZA (USA) — SHIPPING POLICY

Effective Date: 07/01/2026

This Shipping Policy describes how Medaza, Inc. ("Medaza," "we," "us," or "our") handles shipping, duties, taxes, insurance, and customs clearance for Orders placed through the Medaza (USA) Platform. Shipping terms differ between domestic (United States) and international shipments, as described below.

1. Domestic Shipments (within the United States)

  • Sales Tax. Estimated applicable sales tax is calculated and collected from Buyer at checkout, based on the shipping destination and applicable state and local tax rates.
  • Shipping Cost. The cost of shipping is calculated and collected from Buyer at checkout.
  • Insurance and Signature Requirement. Medaza covers the cost of shipment insurance and mandatory signature-on-delivery for all domestic shipments, at no additional cost to Buyer. A signature from an authorized recipient is required at the time of delivery.
  • No Customs or Import Fees. Domestic shipments do not involve customs clearance, import duties, or related fees.

2. International Shipments

For Orders shipped internationally, Medaza acts as the Importer of Record (IOR) and manages customs clearance directly, so that Buyer does not need to separately arrange for import clearance, brokerage, or IOR designation. Costs and responsibilities are allocated as follows:

  • Sales Tax. Estimated applicable sales tax (where applicable to the transaction) is calculated and collected from Buyer at checkout.
  • Estimated Duties. Estimated import duties are calculated and collected from Buyer at checkout, based on the applicable Harmonized Tariff Schedule classification, country of origin, and destination country requirements at the time of Order. Actual duties assessed by customs authorities may vary from the estimate; Buyer remains responsible for any difference between the estimated and actual duty amount, where applicable.
  • Shipping Cost. The cost of international shipping is calculated and collected from Buyer at checkout.
  • Insurance and Signature Requirement. Insurance and mandatory signature-on-delivery for international shipments are collected from Buyer at checkout as part of the shipping charge. A signature from an authorized recipient is required at the time of delivery.
  • Customs Clearance Costs. Customs clearance costs — including brokerage fees and IOR-related compliance costs — are covered by Medaza and are not separately billed to Buyer.
  • Customs Clearance Handling. Medaza, acting as Importer of Record, manages the customs clearance process for the shipment, including filing required import documentation and coordinating with its customs broker and carrier. Buyer is not required to act as importer of record or to independently manage clearance for Orders shipped through this Platform.

3. General Provisions

  • Delivery Estimates. All shipping and delivery timeframes provided at checkout or in Order confirmations are estimates only and are not guaranteed. Actual delivery times may vary due to carrier delays, customs processing, weather, or other factors outside Medaza's control.
  • Regulatory Documentation. For certain medical devices, additional import documentation (including FDA-related filings) may be required and will be handled by Medaza as part of the customs clearance process described above.
  • Failed Delivery / Signature Refusal. If a shipment cannot be delivered due to an invalid address, repeated failed delivery attempts, or refusal to sign for delivery, Buyer may be responsible for additional costs associated with reshipment, return, or storage, as applicable.
  • Order Tracking. Tracking information will be provided to Buyer once an Order has shipped.
  • Split Shipments. Orders containing multiple items may be shipped separately from different fulfillment locations or Suppliers; each shipment may arrive at a different time.
